Adult Mental Health Services Case Manager

DESCRIPTION:

We are looking for a Adult Mental Health Services Case Manager that provides comprehensive and intensive case management services to adults with severe and persistent mental illness with history of homelessness and/or history of hospitalizations. Case management services include linkage to community resources such as housing, medical, educational and vocational services. In addition, provides rehabilitation services that focus on improving daily functioning through an integrated recovery approach that improves daily living skills and supports consumers in meeting their goals.

Essential Duties:

  • Provide targeted case management and mental health rehabilitation services to clients, including linking them to appropriate community resources, as well as assisting with money management, skills training, and independent living skills to improve functioning
  • Assist clients who are homeless in obtaining resources for housing within the community and assisting in obtaining benefits to pay for housing where possible. Assist clients in completing necessary housing applications where appropriate and working collaboratively with housing specialist and administrative team on updates to housing placements for ensuring timely notification of changes/updates to housing status.
  • Work as a collaborative team member with inter and intra-agency staff to assist in stabilizing clients
  • Provide intensive crisis intervention services twenty-four hours, 7 days a week to clients on a rotating basis who are at risk of hospitalization and /or incarceration in an effort to avert hospitalization and /or incarceration.

REQUIREMENT:

Requirements:

  • Must be able to pass California State Department of Justice criminal background investigation, Live Scan and/or fingerprinting.
  • Bachelor’s Degree in Social Work, Psychology or related field preferred.
  • Must have valid California Driver's license and the availability of a car with adequate insurance

EXPERIENCE:

Must have at least two years of experience with a BA degree in the field of mental health/social services; 4 years with an AA degree; 6 years with a HS Diploma

SPECIAL SKILLS AND KNOWLEDGE:

  • Maintain at least weekly face to face contact with assigned clients
  • Ensure clients are involved in establishing goals in the areas of living arrangements, social relationships, constructive use of leisure time, productive activity, medical care, and other areas impacting their functional impairments.
  • Complete periodic outcome measures with clients

EDUCATION:

Must have at least two years of experience with a BA degree in the field of mental health/social services; 4 years with an AA degree; 6 years with a HS Diploma
