Full Service Partnership Mental Health Clinician

We are currently seeking an experienced Field Based Full Service Partnership Mental Health Clinician . The Clinician is responsible for providing community based mental health services to TAY and Adults in our Full Service Partnership (FSP) program. The target focal population of FSP participants consist of individuals with severe and persistent mental illness who have history of and/or are at high risk for homelessness, incarceration and/or institutionalization.

Community Health Worker

This position of Community Health Worker will provide a holistic recovery approach to the Justice-involved population (individuals reintegrating into the community from the Los Angeles County Jail or California State Prison, etc.). They will provide the resources and supports needed to improve overall health, social and economic outcomes of the clients. A client-centered approach will be key in navigating the necessary services this population faces.

Full Service Partnership Case Manager

Under the direction of the Clinical Supervisor, provides comprehensive and intensive case management services to adults with severe and persistent mental illness. Case management services include linkage to community resources such as housing, medical, educational and vocational services. In addition, provides rehabilitation services that focus on improving daily functioning through an integrated recovery approach that improves daily living skills and supports consumers in meeting their goals.

Peer Advocate

Spanish Speaking Mental Health Therapist- Children and TAY

We are currently seeking an experienced Bilingual Clinician/Mental Health Therapist (English/Spanish) who is responsible for providing mental health services to children, TAY youth and families. Services consist of individual and family therapy as well as case management services that may range in intensity depending on need of the family. Therapists perform services which are within the scope of their training and experience.

Adult Full Service Partnership Clinical Supervisor

We are currently seeking an experienced Clinical Supervisor for the Adult Full Service Partnership program. The FSP program is funded by Proposition 63, the Mental Health Services Act. This FSP Program is for adults ages 26-59 who would benefit from an intensive service program for persons diagnosed with a mental illness who would like to work in partnership with a service provider team to develop and achieve individualized recovery-focused goals. Adults in this program have been challenged by homelessness, as well as frequent hospitalizations and/or incarcerations. The best candidate for this position will make it their number one priority to ensure our members are receiving the highest quality of care that can be provided.

Spanish Speaking Mental Health Therapist- Wraparound Program

We are currently seeking an experienced Field Based Spanish Speaking Mental Health Therapist- Wraparound Program. The Wraparound approach is a family-centered, strengths-based, needs-driven planning and service delivery process. It advocates for family-professional partnerships to ensure family voice, choice and ownership. The family, their supports and team work together in order to develop a unique plan to ensure permanency and stability for the youth in his/her home and community. The Child and Family Team Meetings (CFTM) engage families in developing an individualized planning process and formulate out-come based plans using family strengths to assure family voice and choice. The Clinician is responsible for providing community based mental health services to children and families enrolled in the Wraparound program. The target focal population of Wraparound participants consist of children and youth who are at high risk for being placed into a higher level of care and have histories of abuse, neglect and/or incarceration. Services consist of assessments, individual and group therapy, and development of a treatment plan to guide the multi-disciplinary team treating the clients mental health needs. Clinicians perform services which are within the scope of their training and experience.
